Surrey Heartlands Mental Health Programme is inviting expressions of interest from CCG clinical leads working within the field of dementia who may be interested in providing sessional clinical support, advice and leadership to the Health and Care Partnership.
The SH MH programme has two mandates; realisation of the opportunities in the local MH Strategic Framework and delivery of the Five Year Forward View for Mental Health, both of which have a strong focus on improving dementia care and support.
£20k funding is being provided by NHS England for the purpose of making demonstrable improvements in (i) reaching the dementia diagnosis rate (DDR) standard of 67% (ii) provision of post-diagnostic support.
It is anticipated that this will fund 25 PAs over the remainder of 2018/19 for clinical leadership, plus travel expenses. It is a finite, non-recurrent resource for 2018/19 so the project outcomes will need to be deliverable and demonstrable ‘in year’.
This is an exciting opportunity to be involved in spreading best practice across the broader footprint of Surrey Heartlands to improve the services provided to people with dementia and their families.
